1991 - After grounding out, Benito Santiago of the Padres throws his batting helmet in disgust. It bounces into the dugout and hits pitching coach Mike Roarke in the head before ricocheting over to hit coach Greg Riddoch (giving him a concussion). Santiago is fined $300.
